VillageMD - Kentucky helps physicians keep patients healthier and succeed in value-based care

VillageMD, a national primary care provider, has partnered with Primary Care Medical Center, a large independent physician group, to form VillageMD - Kentucky. This new organization will provide solutions and services to its provider partners across Kentucky to improve clinical outcomes, enhance patient and physician satisfaction, and lower the total cost of care.

VillageMD-Kentucky marks the company’s expansion into the Southeast. VillageMD has rapidly grown throughout the Midwest and Northeast to become one of the largest primary care physician networks in the U.S.

VillageMD-Kentucky will leverage VillageMD’s data analytics, physician-based care coordination, and on-the-ground support resources, all of which have demonstrated significantly greater clinical improvements at a lower total cost of care. VillageMD’s clinical care model also aids its physician partners in providing more personalized attention, education, and support to patients via integrated care teams of health coaches, diabetes educators, pharmacists, and resource coordinators to address patients’ medical, emotional and social needs.

“VillageMD’s primary care model is unique in the comprehensive support it provides our physicians to take care of our patients more completely from screening through diagnosis, follow-up, and long term disease management. It is an in-depth coordinated care approach which bridges all the practice and community resources to ensure that nothing falls through the cracks and our patients get the best care they need, said Bob Hughes, MD, Managing Partner, Primary Care Medical Center.

"As we transition to value-based care, VillageMD will be an invaluable partner by giving us access to enhanced contracts, providing a flexible technology platform that easily integrates with our electronic medical record (EMR), and by offering comprehensive patient and practice resources so we are successful in this new area of reimbursement which is based upon quality and value,” said Dr. Hughes.

VillageMD’s high-value model of care is proven to drive results across multiple markets and across all patient populations –commercial, Medicare, Medicare Advantage and Medicaid. Practices that have adopted VillageMD’s patient-centric approach have been recognized by the Centers for Medicare and Medicaid Services for high quality and high performance metrics, such as the following: admissions and readmissions rates 20 to 45 percent lower than the community, Medicare costs 20 to 45 percent lower than the community, and commercial costs 15 to 20 percent lower than the community.

VillageMD’s model has been shown to improve clinical outcomes and lower the total cost of care in chronic disease populations like diabetes where patients have a 93 percent medication adherence rate; hemoglobin A1c control is 63 percent higher, potentially preventable admissions rates are 36 percent lower, and the total cost of their care tracks more than 20 percent lower as compared to community standards.

“Primary Care Medical Center is a very forward-thinking group of physicians who share our philosophy for delivering an enhanced primary care-led model of care. They have delivered best-in-class results and continue to be a great example for primary care groups across the nation, said Tim Barry, chief executive officer, VillageMD. At VillageMD, we wake up every day and work to make the delivery of care easier and better for physicians like Dr. Hughes so they can focus their attention on what matters most-taking excellent care of their patients.”

Established in 1994, Primary Care Medical Center is a nationally recognized physician practice based in Murray, Kentucky which serves the communities located in Southwestern Kentucky and Northwestern Tennessee. The practice specializes in family medicine, internal medicine, pediatrics, and obstetrics and gynecology. They have been recognized as one of the most technology-enabled and comprehensive service offering primary care practices in the nation.
